Having declined a workplace plan you could have taken still counts as having access to it. That catches people out.

CDCP pays its share of the CDCP fee schedule, not of this practice's fee. Where the two differ there can be a balance beyond the percentages above, and the front desk will show you that number before treatment is booked rather than after.
What it covers
Some treatment needs pre-authorisation before it can go ahead. That is a paperwork step, not a refusal, and the office handles the submission.
- Exams, cleanings, scaling and polishing
- X-rays and fluoride
- Fillings and extractions
- Root canal treatment, in some cases
- Dentures, where they are needed
- Sealants for children

Bring that letter to your first appointment. The practice cannot apply on your behalf, but it can tell you whether what you need is covered once you have the number.

Who is eligible for the CDCP?

How much does CDCP pay?
100% of its fee schedule under $70,000 of adjusted family net income, 60% from $70,000 to $79,999, and 40% from $80,000 to $89,999. Because it pays a share of the CDCP schedule rather than of the practice's fee, there can be a balance beyond those percentages, and you will see that number before treatment is booked.
How do I apply for the CDCP?
Through your My Service Canada Account, or by calling 1-833-537-4342. You will need your SIN, address and income information. Sun Life sends a member number and start date if you are approved.
